Ang isubmit ninyo ay inyong katibayan (proof o certification).<\/li><li>Tugkol sa DOLE-AKAP form &#8211; Kung kayo ay walang printer para ito ay maprint, mafill-upan at mapicturan. I-download nyo itong<a href=\"https:\/\/drive.google.com\/file\/d\/1aJbLo1XMJV8b7JQleR_WcM9mgQBYk0dn\/view?usp=sharing\"> PDF file na aming ginawa<\/a> at i-edit nyo ito gamit ang app na Adobe Acrobat Reader na may Fill and Sign Command.
