VACCINATION INSTRUCTIONS

REGISTRATION

Register by entering your Jordanian National Number, or your Personal number if you are non-Jordanian, and your date of birth. Your personal information will appear from the official records.
Specify the healthcare center closest to you, your sector, chronic diseases, and contraindications if any. Sign the consent by clicking the “I agree” box. Enter the mobile number which will receive appointments, notifications, and vaccine certificate messages.
Enter the authentication code you receive on your mobile to activate your file. This information will be used for eligibility and priority.

APPOINTMENT NOTIFICATION

You will receive a message with the details of your vaccination appointment when it is set according to your eligibility and priority.
Priorities are automatically assigned based on risk, benefit, and justice, depending on factors such as age, work sectors, chronic diseases, and others.

ON YOUR APPOINTMENT DATE

Please arrive shortly before the appointment time and bring your identification documents, National ID for Jordanians and passport for others, and bring proof of your affiliation with the chosen work sector (such as a professional ID) and medical reports of chronic diseases (if any).

Please adhere to social distancing and wearing a mask.

The vaccination team will guide you with the instructions for the second dose, depending on the type of vaccine.

VACCINATION CERTIFICATE

When you receive the necessary vaccine doses, you will receive an SMS with the link to your vaccination certificate.

Please save this link for future reference.

REPORT ADVERSE EVENTS

You will receive an SMS with a link to where you can report any side effects which you suspect are associated with taking the vaccine. You can also access this page from your personal profile.

COVID-19 vaccines used in this campaign are safe. Regulations mandate a reporting mechanism for suspected side effects.
